MTA for submitting archival samples to the Repository
NIDDK funded studies that are ready to transfer biological materials to the Repositories must complete the Material Transfer Agreement (MTA) (PDF - 60KB) form and mail to the below listed address. Do not transfer materials to the NIDDK Repository until this document has been signed by NIDDK staff and you have received a copy of the approved Material Transfer Agreement (MTA).
