NIDDK-CR Data and Resources Use Agreement (DUA) - Example Template [ Download PDF - 444 KB ] - The DUA sets all terms and conditions to transfer Resources from NIDDK Central Repository (NIDDK-CR) for an approved Research Project. The DUA is legally binding and signed by the requesting Principal Investigator and institution official agreeing to adhere to the outlined terms of use for the requested resources. This form is an EXAMPLE only and should not be completed and sent to NIDDK-CR. A customized DUA will be generated and routed to you for signature after the request is approved.
NIDDK-CR Cost Recovery and Processing Times [ Download PDF - 103 KB ] - NIDDK-CR charges modest amounts to requestors for the costs associated with retrieving and processing specimens for approved requestors per Section 301.h of the PHS Act (42USC241(h)) and as described in NOT-DK-16-006 and NOT-DK-24-011.

This form is an EXAMPLE only and should not be completed and sent to NIDDK-CR. A customized COD will be generated and routed to you for signature after indicating project closure in your Progress Report.

The document also details the best practices for data security to protect and control access to resources received from NIDDK-CR.
Use of Resources Acknowledgements - A list of acknowledgements that requestors must use when publishing or disseminating results from research using resources from NIDDK-CR. Some contributing studies may require study-specific acknowledgements when citing use of the resources. Data Availability Statements may also be required by some publishers.
